Amy Tracy

Amy Tracy, DNP, PMHNP-BC

Amy Tracy received her Doctor of Nursing Practice degree and Nurse Practitioner license from Fairfield University. Before becoming a nurse practitioner, she was a staff nurse for inpatient and residential mental health services. Amy Tracy is board-certified and has experience as a nurse practitioner in outpatient medication management and therapy, intensive outpatient, and inpatient mental health services.

Amy Tracy is a life span provider, treating ages 2–103. Clinical areas of treatment include depression, anxiety, ADHD, bipolar disorder, OCD, mood disorders, childhood behavioral concerns, learning difficulties, and autism spectrum disorder. She provides a thorough mental health assessment, supportive and educational therapy, and referrals to other providers as necessary to maximize client well-being and overall functioning. “Dr.” is an academic title, not a clinical title. Dr. Tracy does not represent herself as a Psychiatrist, MD, or Physician. Patients may refer to Nurse Practitioner Amy Tracy in any way they feel most comfortable.

Psychiatric Nurse Practitioner

Hello, and Welcome to Swift Creek Mental Health, my name is Pamela Johnson and I have been a nurse for 43 years. 18 years as an ICU nurse and 25-years as a double board certified Adult practitioner as well as Psychiatric Mental Health practitioner. I have been an educator in the field of nursing for over 34 years and have made significant contributions to the healthcare field, specializing in both critical care and mental health. I am a compassionate and skilled clinician, who has demonstrated a lifelong commitment to providing exemplary care to patients across a diverse range of settings.

Early Career and Critical Care Experience

After completing nursing school, I began working in New York Cities Harlem hospital, VA and Bellevue hospital, with a focus on critical care, working in various intensive care units (ICU), including Medical Intensive Care Units (MICU), Coronary Care Units (CCU), and Trauma. With 18 years of hands-on experience in these high-stress, high-stakes environments, I have honed valuable skills and a deep understanding of acute care management and life-saving interventions.

Throughout this time, I have built a reputation for being an effective, calm, and efficient presence in some of the most challenging situations in healthcare.

As an Adult Nurse Practitioner, I have provided primary and acute care, offering services that range from disease prevention and health promotion to the diagnosis and management of chronic conditions. My expertise in adult care, coupled with her extensive experience in critical care, has enabled me to effectively bridge the gap between hospital-based and outpatient care for patients with complex health needs.

Psychiatric Nurse Practitioner certification further underscores my holistic approach to care.

By integrating mental health assessments, treatment plans, and medication management into my practice, I work closely with individuals suffering from mental health disorders, offering a comprehensive and empathetic approach to treatment.
