Please verify that the provider you are seeing at Swift Creek Mental Health Services is in-network with your mental/behavioral health benefits. Many companies have one insurance for medical benefits and separate insurance for mental/behavioral health benefits. You will be responsible for the entire visit cost if we are not in-network with your insurance. We will attempt to verify coverage for you, but you should not consider this a guarantee of coverage. If we cannot verify your coverage before your appointment, you can reschedule or pay the self-pay rate until your insurance is verified.
Medicare for the elderly and retired (Amy Tracy PMHNP-BC accepts a limited amount of Elderly Medicare patients, mostly by referral from community primary care providers, please check with the office main line option 2 to discuss eligibility). We do not provide care for patients with developing dementia-related disorders. Medicare for younger disabled patients may go ahead and schedule via our website. We are not in-network with Medicaid and cannot accept self-pay from patients that are enrolled in Medicaid or Market Plan Policies that we are not in network with.
